Analgosedation First Principle: Prioritize opioid analgesia (Fentanyl 0.5-1 mcg/kg/hr, max 500 mcg/hr ceiling) before hypnotic sedatives to reduce mechanical ventilation duration and delirium.
Sedation Depth: Target light sedation (Richmond Agitation-Sedation Scale [RASS] -2 to 0) rather than deep sedation unless specifically indicated (e.g. severe ARDS, open abdomen, status epilepticus).
