ATLS 11th Edition (2025/2026) / ABA Consensus Formula: Modified Brooke formula is first-line ($2\text{ mL} \times \text{kg} \times \%TBSA$ for adults, $3\text{ mL} \times \text{kg} \times \%TBSA$ for pediatrics $\le 14\text{y}$ / $\le 30\text{ kg}$, $4\text{ mL} \times \text{kg} \times \%TBSA$ for electrical burns). 1st-degree erythema is strictly excluded from %TBSA calculation.
Timing & Maintenance: Half of 24-hour resuscitation volume is infused over the first 8 hours calculated from time of injury (not ED arrival). For children $\le 30\text{ kg}$, add maintenance fluid (D5LR via 4-2-1 rule) to prevent hypoglycemia.
Cyanide Toxicity Antidote: Hydroxocobalamin (Cyanokit) 5 g IV over 15 min for adults (70 mg/kg up to 5 g for pediatrics) indicated in closed-space smoke inhalation with coma, cardiac arrest, hypotension, or lactate ≥ 8-10 mmol/L.
