Wells PE Score & PERC Rule: 2-tier (Unlikely ≤4.0, Likely >4.0). In low-risk/PE-unlikely patients, PERC Rule (all 8 criteria negative) safely excludes PE without D-dimer or imaging (Ann Emerg Med. 2008;52(4):377-383).
ABCD2 Score for TIA: 2-day stroke risk assessment (Age, BP, Clinical features, Duration, Diabetes). High score 6-7 predicts 8.1% 48-hour stroke risk requiring immediate dual antiplatelet therapy and neurovascular admission.
